For that purpose I authorize AmeriVision Communications, Inc./Ameri-Tel to act under my authority as my telecommunications agent in providing "One Plus" long distance service by placing the service orders for the number(s) listed below whenever necessary.   I understand that once my application has been processed and approved that I will become a LifeLine customer.  The Organization will receive funds on domestic Net Paid billing only, no funds are paid on uncollectible billing.
